Altered Passive Eruption

This clinical case presentation demonstrates correction of a gummy smile associated with altered passive eruption. Pre-treatment photographs showed excessive gingival display and short clinical crowns in the anterior maxillary region. Final postoperative photographs demonstrated improved gingival symmetry, increased tooth display, and enhanced smile esthetics following periodontal esthetic treatment.

The case documents esthetic management of altered passive eruption contributing to excessive gingival display (“gummy smile”) in the maxillary anterior region. Initial smile photographs revealed short appearing maxillary anterior teeth covered by excessive gingival tissue, creating disproportionate tooth dimensions and compromised smile esthetics.

Additional close-up intraoral photographs demonstrated altered passive eruption with excessive gingival tissue coverage surrounding the maxillary anterior teeth and asymmetry in gingival architecture.

The treatment approach focused on periodontal esthetic correction to expose additional natural tooth structure, harmonize gingival margins, and improve smile symmetry. Sequential “pre” and “post” images illustrated:

  • Reduction of excessive gingival display
  • Improved clinical crown length
  • Enhanced gingival symmetry
  • Better tooth proportion and smile esthetics
  • Harmonization of anterior gingival contours

Final smile photographs demonstrated significant esthetic improvement with balanced gingival contours and more ideal anterior tooth proportions.
